In order to process your file, the following documents are required Pursuant to Florida State Law.

 

HOME HEALTH AIDES (HHA) & CERTIFIED NURSING ASSISTANT (CNA)

 

* Level two (2) Background Screening

* Current government issued identification with picture (document must not be expired)

* Palm Beach County Caregiver ID badge (HHA's only, if looking for referral in Palm Beach County)

* Home Health Aide Certificate or Certified Nursing Assistant License

* Assist with Self Medication Certificate

* HIV AIDS Certificate

* Valid CPR Card

* Physical that was completed within the past six (6) months; caregiver MUST be declared free

of communicable diseases by a Medical Doctor (MD) or Advanced Registered Nurse Practitioner (ARNP)

* Authorized document to work in the United States

* Professional Liability Insurance

 

**All Documents MUST be ORIGINAL

 

REGISTERED NURSES (RN) & LICENSED PRACTICAL NURSES (LPN)

 

* Level two (2) Background Screening

* Current government issued identification with picture (document must not be expired)

* Nursing License

* Valid CPR Card

* Physical that was done within the past six (6) months; caregiver MUST be declared free of

communicable diseases

by a Medical Doctor (MD) or Advanced Registered Nurse Practitioner (ARNP)

* Authorized Document to work in the United States

* Professional Liability Insurance
